为了优化体检流程，节省体检人员现场填表及等候时间，现将一些常用表格上传供大家下载，建议来前填好表格，填写须知如下：

出境体检人员请到首页“业务指南”中”出境人员体检”下载的相应表格并打印后填写。其中“出入境人员健康检查申请表”参照“样表”如实填写；“新冠流行病学史问卷”为新冠疫情防控需要，请如实填写；填表时间请填体检当天时间。
